Need help with setting up a lan party

Ok so I have 3 xboxes and I'm looking to setup a halo 2 lan party. I know it can go up to 12 players with 3 xboxes, I just have a few questions.

1. Do you need a separate copy of the game for each console?
2. How are you supposed to connect them? Do you need a specific adapter or something? Do you need to connect it to a network?
3. Best maps for lots of people?
