No sound through TV, but sound through headset

Hi all,
For the last few weeks I've been having intermittent issues where I'll be watching Netflix then want to change to say Disney+ and the sound stops coming through the TV. No menu popping noises, nothing.
Sometimes it will be from playing a game and changing etc.
The whole time, if I plug a headset in to the controller, there is audio in that. But as soon as I take it out, the sound is gone.
I've tried:
  1. Hard reset
  2. Unplugging and checking HDMI lead. 3.Changing in volume and audi output settings to stereo uncompressed for HDMI and Optical audio
  3. Changing video fidelity and over scan from auto detect to HDMI.
  4. Shouting at the xbox to work properly.
  5. Updating controller
  6. Cleaning 3.5mm headphone jack slot in controller.
  7. Ensuring xbox is up to date.
I really am at a complete loss here. This thing had worked fine for a few years, then this problem started intermittently occurring and now it seems to be permanent.
Any ideas?
